Overview
This 75-minute on-demand session from the ASAM 55th Annual Conference addresses how to individualize methadone induction dosing in the fentanyl era to stabilize patients.
Opioid treatment programs (OTPs) are governed by regulations at the federal and state level, in addition to individual OTP rules. These can create barriers to access, stabilization and retention. We will review the data and updated SAMHSA recommended flexibilities that allow for more individualized and evidence based care. Attendees will leave with the tools to modernize their approach to providing methadone in their OTPs, ultimately moving away from harmful policies and practices, to safe, effective and patient centered care.
The target audience for this Advanced level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

Learning Objectives
Upon completion, learners will be able to:
- Explain how methadone induction dosing in the fentanyl era needs to be individualized to stabilize patients
- Summarize SAMHSA's newest guidance and be able to apply these changes to daily practice in an OTP
- Evaluate the risks and benefits of increased take-home doses of methadone to improve health outcomes for patients

Ruth A. Potee
MD, FASAM
Dr. Ruth Potee is a board certified Family Physician and Addiction Medicine physician who works in western Massachusetts. She attended Wellesley College, Yale University School of Medicine and did her residency at Boston University where she remained an assistant professor of Family Medicine for eight years. She is currently the Medical Director for the Franklin County House of Corrections, the Director of Addiction Services for Behavioral Health Network, and the medical director for the Pioneer Valley Regional School District as well as the medical director the Opioid Task Force of Franklin County and the North Quabbin Region. She was named Franklin County Doctor of the Year by the Massachusetts Medical Society in 2015.
Laura Kehoe
MD, MPH, FASAM
Laura Kehoe, MD, MPH, FASAM is the medical director of the Massachusetts General Hospital Substance Use Disorder Bridge Clinic, an immediate access, low barrier transitional clinic, which is a component of MGH’s Substance Use Disorder Initiative. She is an Assistant Professor of Medicine at Harvard Medical School. She received her A.B. from Washington University in St. Louis, her MD from Tufts University School of Medicine, and her MPH from Boston University School of Public Health. She completed her residency in internal medicine at Massachusetts General Hospital and is board certified in both internal medicine and addiction medicine. She has cared for patients in primary care, inpatient, residential, and outpatient addiction treatment settings and as a medical director at a Boston area methadone maintenance treatment program.

Dr. Zoe Weinstein
MD, MS
Zoe M. Weinstein MD, MS is an Assistant Professor of Medicine at Boston University School of Medicine, a graduate of Boston University’s Addiction Medicine Fellowship and board certified in Internal Medicine and Addiction Medicine. Her current clinical work and research are focused on long-term Office-Based Addiction Treatment (OBAT) with buprenorphine, and the development and evaluation of inpatient addiction consult services. She has been the director of Boston Medical Center’s Addiction Consult Service since July 2016.
