Overview

This 75-minute on-demand session from the ASAM 54th Annual Conference addresses practical ways to increase access to SUD treatment and MOUD in post-acute care settings.

This interactive workshop will highlight practical ways to increase access to SUD treatment and MOUD in post-acute care settings. Led by a multi-disciplinary group including a hospitalist, addiction consult provider, OTP medical director, and skilled nursing facility (SNF)-based peer recovery coach, the panel will review partnerships to increase access to MOUD and recovery supports at SNFs, including OUD MEETS. Participants will work in break-out groups to brainstorm how to improve access to OUD treatment across into their local contexts.

The target audience for this intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

Learning Objectives

Upon completion, learners will be able to:

  1. identify barriers to integrating OUD treatment into post-acute care.
  2. describe one model of hospital, OTP and SNF partnership.
  3. develop a plan to improve transitions of care for patients with OUD in their practice settings.

Dr. Megan Buresh, MD

Assistant Professor of Medicine, Johns Hopkins School of Medicine

Dr. Megan Buresh is an Assistant Professor of Medicine and Epidemiology at the Johns Hopkins University School of Medicine.  She is board certified in internal medicine and addiction medicine and certified as AAHIVM HIV Specialist.  She received a Bachelor of Arts in Biochemical Sciences from Harvard University (magna cum laude with highest honors in field), completed medical school at Johns Hopkins University School of Medicine and residency at Brigham and Women’s Hospital.  She is the Secretary of the Maryland-DC ASAM Chapter.

She is medical director of the Johns Hopkins Bayview Inpatient Addiction Consult Service.  She also provides integrated buprenorphine treatment as part of her outpatient primary care practice.  She is lead physician for BHLI PCARE, a mobile buprenorphine clinic for patients being released from Baltimore City Jail.  Her research interests include public health and addiction, developing and evaluating models to expand office-based opioid treatment, and improving linkages between clinical addiction treatment and community organizations.  She is a co-investigator with the ALIVE study at the Johns Hopkins Bloomberg School of Public Health, where she is studying risk factors for opiate overdose and access to naloxone among persons who inject drugs.    

Cat Chamberlain, MD

Dr. Chamberlain is a family physician and addiction medicine specialist in Baltimore, MD. She works part time as a medical director of 7 opioid treatment programs and part time at an FQHC combining medication assisted treatment with primary care. She went to medical school at Brown University and completed her family medicine residency in 2017 at University of Maryland in Baltimore. She went through the practice pathway and became board certified in Addiction Medicine in 2021. Her interests include expanding access to care for patients seeking medication assisted treatment and low barrier treatment. Outside of work, she enjoys spending time with her family and training for half marathons.

Consuelo Green CPRS, RPS , Certified Peer Recovery Specialist for Johns Hopkins Bayview. I am part of the OUD pilot team at Johns Hopkins Bayview. My role as a Certified Peer Recovery Specialist is to meet with the patient on the inpatient unit, start by introducing myself and share a little about my story. I have been in recovery from all drugs and alcohol for over 5 years and the steps I took to acheive this. After I share with them about myself I then start finding out if the patient have any recovery goals and how I can help. During this conversation I can determine the patient readiness and desire for treatment and recovery. At this point, I encourage and support any life style changes the patient desire concerning recovery from drugs and alcohol. I understand that their are many pathways to recovery so I stay patient centered to what ever goals the patient have. I also inform the patient about the Wellness and Recovery groups that I facilitate at the SNF that they will be going to. Once at the SNF I have one on one meetings with the patient to keep them engage in recovery and plan future aftercare goals. I relay this information with the team to keep updates on the patient during weekly conference calls and emails. My role as a Peer is to encourage, motivate, mentor and support Johns Hopkins Bayview patients going to Skilled Nursing.
