Overview
This 75-minute on-demand session from the ASAM 55th Annual Conference addresses key factors and common pitfalls in 3.7R program development, including well-defined admissions criteria, collaboration between medical and behavioral health staff, and careful attention to care transitions.
Medically monitored residential care fills a critical gap in the SUD continuum of care, providing residential services to people with complex medical and psychiatric conditions. This interactive workshop will review the development of a 3.7R program, from initial vision to payer negotiations to program implementation and expansion. We will discuss lessons learned and provide resources including admissions criteria, case management tools and documentation templates. We will review common challenges to developing 3.7R care and how to overcome them.
The target audience for this Intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

Learning Objectives
Upon completion, learners will be able to:
- Describe the critical need for 3.7R, medically monitored residential services in the United States and how these services fill an important gap in the SUD continuum of care
- Identify key factors and common pitfalls in 3.7R program development, including well-defined admissions criteria, collaboration between medical and behavioral health staff, and careful attention to care transitions
- Examine challenges to development of 3.7R programs in participant's communities and strategies to overcome these challenges, including community partnerships, outcomes drive program design and assessment of return on investment

Eowyn Rieke
MD, MPH
Eowyn Rieke MD MPH is a family doctor, addictions medicine specialist and medical leader. She has worked for over 20 years in health care for homeless and other marginalized groups. She is dedicated to person-centered trauma-informed care that supports the dignity and self-determination of her clients, working collaboratively with them towards their goals related to substance use and other aspects of their lives.
Eowyn has devote the last 5 years of her career to expanding integrated care across health discipines and low-barrier access to buprenorphine, first as the Services Director of Central City Concern's Blackburn Center, and currently as the Medical Director for Outpatient Services at Fora Health in Portland, Oregon. At Fora Health she is developing programs to provide medications for OUD to Latinx patients and rural residents of Oregon, as well as improving transitions of care from withdrawal management and residential programs to outpatient medical services.
Eowyn is vice-president of the Oregon chapter of the American Society of Addiction Medicine and is a member of the Oregon Measure 110 Oversight and Accountability Council. She has served as chair of the National Health Care for the Homeless's Clinciian Steering Committee and on the NHCHC Board of Directors.
Eowyn earned her MD degree from Brown University's Warren Alpert Medical School and her Masters in Public Health from Harvard University. Eowyn loves to spend time where there are more trees than people, especially with her daughter, partner, and rascally dog Huckleberry Finn.

Charlotte VanCleve, MSN, PMHNP

Fora Health
Charlotte VanCleve is a PMHNP and received her Master of Nursing at Vanderbilt University. She has worked at Fora Health as a PMHNP since 2019, working in outpatient and residential substance use care. She teaches learners from Vanderbilt and OHSU, including PMHNP nursing students, Addiction Fellows, and Addiction Psychiatry Fellows. Charlotte started working at Fora (formerly known as DePaul Treatment Centers) at the front desk in 2014, and worked nights in withdrawal management as a tech while working on nursing pre-requisites before eventually attending graduate school. Charlotte loves going on hikes with her dog, Professor Biscuit.

Daniel Sessions, MD, FACMT, FAAEM
Senior Lecturer
University of Queensland, Ochsner Clinical School
Dr. Daniel J Sessions MD is the Chief of the Division of Medical Toxicology at Ochsner Health Foundation. He is a senior lecturer in Emergency Medicine at the University of Queensland, Ochsner Clinical School in New Orleans, LA. He received his medical doctorate from the LSU-New Orleans School of Medicine. He was a Chief Resident in Emergency Medicine during his training at Brooke Army Medical Center in San Antonio, TX. He completed his medical toxicology fellowship at Rocky Mountain Poison & Drug Center at Denver Health.
After completing his training, Dr. Sessions returned to Brooke Army Medical Center where he served as the Director of Medical Toxicology. His Graduate Medical Education service included: Research Director of the Emergency Medicine Residency, Associate Program Director in Emergency Medicine, and Transitional Year Internship Associate Program Director. He served as a consulting toxicologist for the South Texas Poison Center, Army Substance Abuse Program, and Dept. of Defense ADVISOR system. After completion of his military service, he became board-certified in Addiction Medicine and practiced Addiction Medicine and Toxicology at Abbot Northwestern Hospital and Minnesota Poison Control in Minneapolis, MN.
He is currently a senior lecturer in Emergency Medicine at the University of Queensland, Ochsner Clinical School in New Orleans, LA, and part of the Emergency Medicine Residency Faculty. He provides Forensic Toxicology services through Expert Toxicology, LLC. His primary areas of interest are addiction medicine, envenomation, drug testing, and performance-enhancing drugs and supplements.

Physicians
The American Society of Addiction Medicine designates this enduring material for a maximum of 1.25 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

PAs
