Overview

This 75-minute on-demand session from the ASAM 55th Annual Conference addresses challenges and barriers to developing a clinical pathway for withdrawal management in a children's hospital.

A clinical pathway specific to youth is a critical part of safely managing symptoms of withdrawal from illicit use of drugs and alcohol in an acute medical setting. A well-designed, collaborative pathway can improve medical and behavioral health outcomes. This session will provide insights into creating an institution-wide withdrawal pathway in a children's hospital. This dynamic workshop includes interactive polling, a panel discussion, and brief didactic on creating a framework to build a withdrawal pathway at your own institution.

The target audience for this Intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

Krishna White, MD, MPH, FASAM

Attending Physician

Children's Hospital of Philadelphia

Krishna White, MD, MPH is an attending physician in the Craig-Dalsimer Division of Adolescent Medicine at Children's Hospital of Philadelphia and an Associate Professor of Clinical Pediatrics in the Perelman School of Medicine at the University of Pennsylvania where she serves as Medical Directors of Substance Use Services and Outpatient Clinical Operations for the Adolescent Medicine Division. She is board certified in Pediatrics, Adolescent Medicine, and Addiction Medicine. She has interests in reproductive health including provision of long-acting reversible contraceptive methods and adolescent and young adult substance use disorders.

Dr. White attended Hampton University, a HBCU in Hampton, VA. She graduated from Eastern Virginia Medical School in Norfolk, VA and then completed pediatric residency at the Children’s Hospital of Philadelphia. She trained in Adolescent Medicine at Children’s National Medical Center in Washington, DC. During her fellowship she also received a Master of Public Health in Health Policy from The George Washington University. After fellowship she started the Adolescent Medicine Division and became Chief of Adolescent Medicine & Pediatric Gynecology at Nemours/Alfred. I DuPont Hospital for Children in Delaware. In 2019, she joined the Division of Adolescent Medicine at CHOP and became the Medical Director of Substance Use Services. She also serves as an EPIC Super User and Clinical Champion for Patient Portal Privacy. After years of clinical practice and advocacy, she became board certified in Addiction Medicine in 2021 via the practice pathway. In 2022, she became the Medical Director for Adolescent Medicine Outpatient Clinical Operations.

Dr. White also a proud wife, mother of a teen daughter, and stepmother of three young adults in college and lives in New Castle, Delaware.

Rachel Linstead Goldsmith, MPA

Program Manager

Children's Hospital of Philadelphia

Rachel Linstead Goldsmith, MPA is the Comprehensive Opioid Response and Education (CORE) Program Manager for the Center for Public Health Readiness and Response at Children's Hospital of Philadelphia (CHOP), which is also home to the Poison Control Center at CHOP. The CORE Program operates as an enterprise-wide forum for communication, coordination and collaboration to address the opioid/overdose epidemic and its impacts in the region. Prior to joining CHOP, Rachel has worked on public health campaign and programs to increase contraceptive access, assist and enroll the public in Affordable Care Act health coverage, assure quality community services for older adults, improve care standards and support research for adults with congenital heart disease, and support people living with HIV/AIDS through treatment, research and social services.

Frank P. James

MD, JD, FASAM

Frank P. James, MD, JD, FASAM, FACLM

Dr. James earned a Bachelor of Arts in Premedical Studies and Japanese Language from the University of Notre Dame.  He earned his Law and Medical Degrees at Southern Illinois University.  Dr. James is a member of both the Ohio and Illinois Bar.  Dr. James is a Veteran of the United States Army, having completed Residency in General Psychiatry at Walther Reed Army Medical Center.  He then completed Fellowships in Child and Adolescent Psychiatry at The Children’s Hospital of Philadelphia and Forensic Psychiatry at the University of Cincinnati School of Medicine.  He is Board Certified in General, Child and Adolescent and Forensic Psychiatry as well as Addiction Medicine.
Dr. James spent his clinical years providing inpatient and outpatient psychiatric services in underserved areas in the Ohio Valley.  He developed specialty clinics in ADHD, Bipolar Disorder and Schizophrenia, and developed an Opioid Use Disorder (OUD) outpatient clinic focused on the integration of group therapy, psychotropic medication management, and Medication Assisted Treatment (MAT). 
Dr. James has over five years of experience in managed care with expertise in OUD clinical program development, provider network expansion, and quality improvement measures.  Through participation in peer case discussion, complex case coordination, fraud, waste and abuse investigation, and utilization management, Dr. James provides large behavioral health organizations guidance in drafting evidenced-based benefit guidelines and prior authorizations specific to OUD treatment and MAT.  With an emphasis on understanding member needs, collaborating with providers, and improving payer relations his current passion is medical/behavioral integration and alternative payment models. 
Dr. James is a member of ASAM’s Finance Committee, Vice Chair of ASAM’s Payer Relations Committee, and Alternate to ASAM’s Board of Directors for Region III.

Amanda Smith, MPH

Epidemiologist

Centers for Disease Control and Prevention

Amanda J. Smith, MPH is an epidemiologist at the U.S. Centers for Disease Control and Prevention. Last year she joined CDC's Division of Overdose Prevention after 20 years in the Division of HIV Prevention. Her expertise includes conducting bio-behavioral surveillance using health questionnaires and collecting multiple specimen types among communities experiencing health disparities such as persons who inject drugs.
