Overview
This 75-minute on-demand session from the ASAM 54th Annual Conference addresses models of care navigation for successful transitions between partner organizations.
Denver Health Medical Center and Sobriety House collaborated to lower barriers and improve outcomes for clients transitioning from hospital into high-quality residential treatment. In this presentation, we will discuss our work building a sustainable partnership and share data demonstrating improvement in linkage from hospital to residential SUD treatment and integration of medical (i.e., COVID-19 testing/treatment and intravenous antibiotics) and psychiatric services during residential treatment. Small breakout sessions with the presenters will be included.
The target audience for this intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

Hermione Hurley, MBChB is an Infectious Disease, Addiction Medicine physician at Denver Health and Assistant Professor of Medicine at the University of Colorado. Her interest in substance care began after observing poor health outcomes for justice involved individuals at Denver Health Correctional Care Medical Facility. Her subsequent decision to pursue fellowships in Infectious Disease and then Addiction Medicine was motivated by the high prevalence of infectious disease and substance use disorders among individuals who are recently released from jail or prison. Her current position at Denver Health is supported by both Psychiatry and Medicine departments to increase treatment for individuals irrespective of where they seek care, be it the ID or methadone clinic.

My name is Jennifer Naeger, I am a Licensed Clinical Social Worker (LCSW) and a Licensed Addiction Counselor (LAC). I currently work at Denver Health Outpatient Behavioral Health Services in the role of Treatment on Demand Program Supervisor, a team that completes behavioral health intakes and linkage to care, within both the Emergency Department and Inpatient setting, for patients with a diagnosed substance use disorder. I have worked in the behavioral health field for over fifteen years and have been with Denver Health for the last six years. I completed my undergraduate degree in psychology at Fontbonne University in 2004 and my graduate degree in social work at University of Chicago in 2007. My areas of interest are: increasing and improving access to behavioral health treatment, working with under-served communities, as well as integrated models of care.

Crystal Ahles, LCSW, LAC, CNP, is Executive Director of Sobriety House, Inc. in Denver, Colorado. She began her early career in corporate America after obtaining her B.S. in Finance, and in 2008 changed direction with a desire to serve others and make a difference in the community by pursuing her MSW at the University of Denver. Through internship, she entered the field of residential substance use disorder treatment and has been working with this population (adults, primarily indigent) for the past 12 years. She has worked for her current organization for 10 years. Originally a full-time counselor, Crystal had the honor and privilege of becoming Deputy Director and succession planning with former Executive Director, Bill Dolan, who served Sobriety House for 40 years. He retired in the spring of 2021. She is learning and growing as a leader every day and feels immense gratitude for the team that makes Sobriety House such a quality provider of SUD services.
