Overview

This 1-hour, on-demand session from the ASAM 53rd Annual Conference discusses opioid use disorder treatment during pregnancy and in the postpregnancy period.

The twin crises of Covid-19 and opioid overdose mortality pose unprecedented challenges to birthing people with substance use disorders, their families, and the providers caring for them. This session provides a review of evidence and strategies to help providers navigate these challenging times, including public-health research updates for treating pregnant people with OUD, strategies to initiate buprenorphine, and methadone in pregnant people using fentanyl, and adequate dosing of methadone during pregnancy using plasma levels and flexible dosing schedules.

The target audience for this Introductory level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

This session addresses the following ACGME Competencies: Patient Care and Procedural Skills, Medical Knowledge, Practice-based Learning and Improvement, Interpersonal and Communication Skills, Professionalism, System Based Practice.

Learning Objectives

Upon completion, learners will be able to:

  1. identify current national data related to COVIDSUD and available resourcesand will appreciate tools for access to care for birthing people with substance use disorder.
  2. gain knowledge of maternal mortalityoverdose related to methamphetaminefentanyl/OUD during COVID and appreciate ways to improve comprehensive and lifecourse care. practices.
  3. appreciate buprenorphine and Methadone pharmacokineticsinitiation and crossover protocolsand flexibility of treatment (in-person and telehealth) settings during COVID.

Vania Rudolf

MD, MPH, DFASAM

Vania Rudolf, MD, MPH, DFASAM is a family and addiction medicine physician who is providing care to pregnant and parenting women with substance use disorder. She has a background in family medicine with OB and has completed Fellowships in Integrative Medicine, Addiction Medicine and High Risk Obstetrics at Swedish Medical Center. She has earned public health training at the UW. Dr. Rudolf continues to be a compassionate advocate for improving kind non-judgmental services and removing barriers to care. Providing compassionate trauma-responsive care to vulnerable women, babies and families is the primary focus of her practice in addition to clinical research related to clinical outcomes for substance use disorder, transition to MAT and prescribing policies to optimize safe and adequate postpartum analgesia management. She is the Medical Director for Addiction Recovery Services at Swedish Medical Center, an assistant professor at University of Washington, Seattle, WA and the Chair for the National Womens' Addiction Group related to the American Society of Addiction Medicine. She serves on the maternal mortality review board for the state of Washington and has many years as a practicing clinician.

Ngoc Pham, MD is an addiction medicine fellow at Swedish Medical Center in Seattle. After completing a family medicine residency, she completed a high risk advanced OB fellowship under the Maternal Fetal Medicine service in Seattle. Her goes are to care for substance use in pregnancy as well as to focus on homeless healthcare.

Tricia Wright

MD, MS, FACOG, FASAM

Tricia Wright, MD MS is a Professor of Clinical Medicine in the Department of Obstetrics, Gynecology and Reproductive Health at the University of California, San Francisco. Previously she was at the University of Hawaii and founded the Path Clinic, a perinatal clinic specializing in the care of pregnant and parenting women with Substance Use Disorders. She is board certified in both Obstetrics and Gynecology and Addiction Medicine and a Fellow of the American College of Obstetrics and Gynecology and a Distinguished Fellow of the American Society of Addiction Medicine. She has published multiple papers on pregnancy and addiction as well as a textbook Opioid Use Disorders in Pregnancy published in 2018 by Cambridge University Press.Dr. Wright completed her undergraduate degree in Biological Sciences from Stanford University, her MD from the University of Michigan. She completed her residency in Obstetrics and Gynecology from the University of New Mexico and obtained a Master’s Degree in Clinical Research from the University of Hawai‘i.

Hendree E. Jones

PhD

Hendree Jones, PhD is an internationally recognized expert in the development and examination of both behavioral and pharmacologic treatments for pregnant women and their children in risky life situations. She has received continuous National Institutes of Health funding since 1994 and has written more than 200 publications. Dr. Jones has also authored two books, one on treating patients for substance use disorders and the other on comprehensive care for women who are pregnant and have substance use disorders. She also has written multiple textbook chapters on the topic of pregnancy and addiction. Dr. Jones has co-authored multiple national and international guidelines on the topic of caring for pregnant and post-pregnant people with substance use disorders and their children including those published by the World Health Organization (WHO), the Substance Abuse and Mental Health Services Administration and the American Society for Addiction Medicine. She also co-authored both the women’s and children’s section of the United Nations (UN)’ International Standards for the Treatment of Drug Use Disorders and the UN guidelines on prevention and treatment for girls and women. While winning multiple awards, most recently in 2020 Dr. Jones won the American Society of Addiction Medicine (ASAM) R. Brinkley Smithers and Distinguished Scientist Award. She is a consultant for the UN and the WHO and is a member of the NIH’s HEAL multidisciplinary working group. Dr. Jones leads or is involved in projects around the world focused on improving the lives of children, women, and families.
