Overview
This 1-hour, on-demand session from the ASAM 53rd Annual Conference describes the usage of the free, paper-based interview form developed for the ASAM Criteria.
The ASAM Criteria Intake Assessment is intended as a guide to multidimensional assessment of individuals with Substance Use Disorders (SUDs) and as a guide to the conceptual approach of the ASAM Criteria decision logic. It is designed to assist Level of Care (LOC) decision-making for adults who are seeking treatment. The ASAM Criteria Intake Assessment Form for Adults has two primary components. The first component is a set of questions that guides the interviewer to establish risk ratings for the six domains of The ASAM Criteria. The second component of the form includes a summary sheet and Level of Care Guide where the interviewer summarizes information from the interview questions and uses the risk ratings from the six dimensions and guidance embedded in the assessment form to select the appropriate LOC. This session will also describe the development of the tool and the results of a small feasibility study. Additionally, this session will provide an overview of the assessment, discuss scoring guidance, and explore information and resources for anyone who would like to implement the tool locally.
The target audience for this Introductory level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.
This session addresses the following ACGME Competencies: Patient Care and Procedural Skills.

Anne Lee, LCSW
Project Director
Anne B. Lee, LCSW, is a Project Director at the UCLA Integrated Substance Abuse Programs (ISAP). She received her MSW at Smith College School for Social Work and a Bachelor of Arts at Wesleyan University. At UCLA ISAP, she serves as study Project Director and therapist for research involving people with co-occurring substance abuse and mental disorders. Ms. Lee is Clinical Content specialist for the California Drug Medi-Cal Organized Delivery System (DMC-ODS) Waiver evaluation. Prior to joining UCLA ISAP, she served at the UCLA Neuropsychiatric Institute as Project Director for The TORDIA project (Treatment of Resistant Depression in Adolescents), a multi-site study of drug and talk therapy treatment regimen for severely depressed adolescents. Before coming to UCLA, she served at Harvard Medical School as Project Director and Clinical Interviewer for The Family Pathways Project, a multigenerational longitudinal study of high-risk parent-child attachment. Prior to her work in research, she was a Social Work Fellow at The Boston Children’s Hospital in the Department of Psychiatry.

R Corey Waller
MD, MS
R. Corey Waller MD, MS, FACEP, DFASAM is a board-certified and actively practicing addiction and emergency medicine specialist with extensive experience working at the local, state, and national levels. He has works with the American Society of Addiction Medicine (ASAM) and related organizations to advance the evidence-based practice of treatment and recovery for individuals affected by substance use disorder (SUD). A nationally recognized expert in SUD, he is now the Editor in Chief of The ASAM Criteria.
As a principal at the HMA Institute on Addiction, he is directly responsible for consultation regarding addiction treatment system development for hospitals, primary care practices, just involved populations, and addiction treatment providers. He oversees the HMAedu.com learning management system and continues to develop education covering addiction, pain, behavioral health, and the administrative support for those systems.
In his prior role as senior medical director for education and policy at the National Center for Complex Health and Social Needs, Dr. Waller was responsible for developing and maintaining all training and in-person technical assistance delivered by the National Center. This covered addiction, pain, behavioral health treatment system development, correctional medicine, payment model implementation, and healthcare policy.
Before joining the National Center, he worked for the Spectrum Health System in Grand Rapids, Michigan, a fully integrated health system with 14 hospitals and more than 1,000 employed physicians. Dr. Waller served as medical staff chief of pain medicine to the Spectrum Health Hospital System and medical director of the Spectrum Health Medical Group Center for Integrative Medicine, where he was responsible for treating patients with addiction, including management of pregnant mothers with substance use disorders. Additionally, he oversaw the development of core competencies for addiction treatment in the labor and delivery unit and the neonatal abstinence syndrome treatment algorithm for the Level 1 neonatal intensive care unit.
