Overview

This 1-hour, on-demand session from the ASAM 53rd Annual Conference covers alternatives to benzodiazepines for treatment of withdrawal, including phenobarbital, gabapentin, and other options.

Benzodiazepines are the mainstay of treatment for alcohol and other sedative-hypnotic withdrawal syndromes. In some instances, benzodiazepines may not be appropriate for treatment of withdrawal, such as cases of chronic abuse of benzodiazepines, withdrawal from both alcohol and benzodiazepines, and the use of ?-hydroxybutyric acid (GHB). Alternative pharmacologic options are available, including phenobarbital, gabapentin, alpha-adrenergic agonists, and others. This presentation draws from existing medical literature and includes expert, clinical experience with complicated patients. This session incorporates an evidence-based review of clinical issues, such as assessment of withdrawal, choosing appropriate medication regiment, monitoring severity, triaging to appropriate level of care, and addressing related medical and psychiatric complications.

The target audience for this Advanced level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

This session addresses the following ACGME Competencies: Patient Care and Procedural Skills, Medical Knowledge.

Learning Objectives

Upon completion, learners will be able to:

1. Identify different evidence-based pharmacologic options for treatment of withdrawal from alcohol and/or sedatives.
2. Recognize risks and benefits of pharmacologic options for sedative withdrawal treatment, including phenobarbital, gabapentin, and other medications.
3. Discuss treatment of complex sedative withdrawal syndromes in different settings.
