Overview

This 75-minute on-demand session from the ASAM 56th Annual Conference addresses emerging substances of misuse and their impact on pregnancy, with guidance on harm reduction, clinical care, and counseling in the context of evolving cannabis and CBD use.

This session will address the perinatal impact of emerging topics related to substance use / misuse. Participants will be presented with key management considerations while caring for pregnant and parenting people presenting with exposure to or use of emerging substance use threats such as xylazine, nitazenes, tianeptine, kratom and cannabidiol (CBD). It will also provide an introduction to newer applications for well-established paradigms of care, such as the development of a perinatal harm reduction model.

The target audience for this Intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

This session addresses the following ACGME Competencies: Patient Care and Procedural Skills, Medical Knowledge, Practice-based Learning and Improvement, Interpersonal and Communication Skills, Systems-based Practice.

This session addresses the following IPEC Competencies: Values and Ethics, Roles and Responsibilities, Teams and Teamwork.

Learning Objectives

Upon completion, learners will be able to:

  1. Identify emerging substances of misuse (i.e. kratom, nitazenes, tianeptine and new supply chain contaminants - e.g. xylazine) and discuss how they affect clinical outcomes for patients with use during pregnancy and postpartum.
  2. Apply up to date evidence on emerging substances for providing optimal clinical care during pregnancy and postpartum and describe the approach to incorporating perinatal harm reduction in prenatal care.
  3. Develop best practices for clinical care in the context of medicinal and recreational cannabis legislation and discuss the approach to patient counseling and management with cannabidiol (CBD) use in pregnancy.

Niraj Chavan, MD, MPH, FACOG, FASAM

Associate Professor, Fellowship Director

Saint Louis University / SSM Health

Niraj R. Chavan, MD, MPH, FACOG, FASAM is a Maternal Fetal Medicine (MFM) and Addiction Medicine physician and holds faculty appointment as an Associate Professor in the Div. of MFM, Dept. of Obstetrics, Gynecology, and Women’s Health at Saint Louis University School of Medicine (SLUSOM). He serves as the Medical Director of the Women and Infant Substance Help (WISH) Center at SLU/SSM Health, focused on managing perinatal substance use disorders (SUD) in St. Louis, MO. He also serves as the Program Director for the MFM fellowship, core faculty for the Addiction Medicine fellowship and Course Director for a SLUSOM course on “Perinatal Addiction and Maternal Mental Health”. Most recently, Dr. Chavan was appointed as the Regional Obstetric Quality Chair for SSM Health – St. Louis, in order to oversee perinatal quality improvement efforts across 5 hospitals in the St. Louis region. He was also recently elected to the position of Secretary for the Midwest Chapter of the American Society of Addiction Medicine (MWSAM).

Dr. Chavan currently serves on the Board of Directors of the American College of Obstetrics & Gynecology (ACOG) as the Early Career Fellow-at-Large and is a member of the ACOG Committee on Clinical Practice Guidelines for Obstetrics, Quality and Safety Initiatives delegation and the Society of Maternal Fetal Medicine (SMFM) – Patient Safety and Quality Committee. He also serves as the Chair-Elect for the Missouri Perinatal Associated Mortality Review (PAMR) Board and the Maternal Health Chair for the Missouri Perinatal Quality Collaborative (MOPQC). Dr. Chavan is Board Certified in Obstetrics & Gynecology, Maternal Fetal Medicine and Addiction Medicine and has lectured extensively on various aspects of perinatal SUD management at the local, regional and national level. He is actively working towards implementation of statewide quality initiatives directed at reducing disparities in maternal mortality and severe maternal morbidity.

Cresta Wedel Jones MD, FACOG, is an associate professor of Maternal Fetal Medicine at the University of Minnesota Medical School, Department of Obstetrics, Gynecology and Women's Health. She serves as the co-chair of the Minnesota Maternal Mortality Review Committee and sits on the Minnesota Governor's Task Force on Substance Use and Pregnancy Health. Her clinical and research focuses include examination of cord blood markers to predict severity of neonatal opioid withdrawal syndrome and reducing bias and stigma in perinatal addiction care.

Maria Manriquez, M.D., FACOG, FASAM, PMH-C is a Professor at the University of Arizona College of Medicine-Phoenix in the Department of Obstetrics and Gynecology. She specializes in care for women with substance use disorders. She is board-certified in both Obstetrics/Gynecology and Addiction Medicine. She is also certified in Perinatal Mental Health by Perinatal Support International. Dr. Manriquez is a current member of the American College of Obstetricians and Gynecologists (ACOG) Fetal Alcohol Spectrum Disorder (FASD) group. The group is committed to educating fellow ACOG members about the risks associated with alcohol and other substances in pregnancy. She is the medical director for the addiction medicine and ambulatory OBGYN at Banner University Medical Center Phoenix and directs the UME education program in Pain and Addiction at the University of Arizona College of Medicine-Phoenix.
