Overview

This 75-minute on-demand session from the ASAM 56th Annual Conference explores replicable community-based models of care that leverage existing infrastructure in transformative ways to address the treatment gap.

While the science and practice of addiction medicine has exponentially expanded over the last few decades, it is estimated that only 10% of individuals with substance use disorders receive any form of treatment in the United States. Challenges from stigma to insufficient treatment access and screening practices have created a widening treatment gap that cannot be addressed by any one approach or model alone. This opening session will explore replicable community-based models of care that leverage existing infrastructure in transformative ways to address the treatment gap. Highlighting creative models across the nation, attendees will be inspired to consider how practical changes to current systems can create meaningful impact and expand access to care.

The target audience for this Introductory level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

This session addresses the following ACGME Competencies:Patient Care and Procedural Skills, Practice-based Learning and Improvement, Systems-based Practice, Interpersonal Communication and Skills, Professionalism.

This session addresses the following IPEC Competencies: Values and Ethics, Roles and Responsibilities, Teams and Teamwork.

Learning Objectives

  1. Describe the challenges and critical considerations in establishing a mobile medical unit for substance use disorder treatment, including the importance of integrating medical care into trusted community-based harm reduction programs, recognizing the limitations of traditional care systems, and addressing the need for comprehensive services beyond medications for opioid use disorder (MOUD).
  2. Identify innovative, patient-centered approaches to delivering care for vulnerable populations, including the use of non-traditional care pathways, mobile health strategies to overcome access barriers, integrated support for social determinants of health, trauma-informed behavioral health services, and agile adaptations to complex service environments.
  3. Describe challenges and successes of innovative interventions to expand access to medications for opioid use disorder and harm reduction resources to people experiencing homelessness.

Dr. Caleb Banta-Green, PhD, MPH, MSW

Principal Research Scientist, University of Washington

Caleb Banta-Green PhD MPH MSW is Principal Research Scientist at the University of Washington’s Alcohol & Drug Abuse Institute and an Affiliate Associate Professor, Department of Health Services UW School of Public Health. His main areas of research are illicit substance use disorder and overdose interventions particularly for non-care seeking populations utilizing syringe exchanges, involved in the criminal legal systems, and/or whom are homeless. He provides technical assistance and evaluation services for public health and safety interventions including the website http://stopoverdose.org. He is also an epidemiologist and reports drug trends across WA State, with data available online http://adai.uw.edu/wadata/ and has been the Seattle area representative to NIDA's drug epidemiology workgroups since 2001. He serves on the U.S. Department of Health and Human Service's Interagency SUD Advisory Coordinating Committee as well as multiple local and state workgroups and committees related to epidemiology and interventions for illicit substance related problems. He served as senior science advisor for the Office of National Drug Control Policy in the Executive Office of the President in 2012.

Avik Chatterjee

MD MPH

Dr. Chatterjee is a med-peds trained primary care and addiction medicine physician at several shelter-based clinics through Boston Health Care for the Homeless Program. His areas of clinical and research interest include innovative treatment models for opioid use disorder in marginalized populations, and interventions on social determinants of health, such as food insecurity. He has an additional interest in racism and health care, particularly its manifestations in medical education and training.

Everett Maroon, MPH

Executive Director

Blue Mountain Heart to Heart

Everett Maroon, MPH, is Executive Director of Blue Mountain Heart to Heart (BMHTH), a 501(c)3 nonprofit based in Walla Walla, Washington, focusing on HIV care and prevention, and care and recovery for people with substance use disorder. He is engaged in regional and statewide efforts to increase pathways for recovery in rural areas and for stigmatized patient populations. Everett co-authored the Greater Columbia Accountable Community of Health's opioid demonstration project, called the Opioid Resource Network, and continues to be engaged in Medicaid transformation efforts in his region. He manages a regional site for naloxone distribution on a multi-year SAMHSA contract, is a co-manager of the first LEAD diversion program in Washington State east of the Cascades, and manages the medical and opioid medication program at the Walla Walla County Jail. He oversees the agency's health engagement hub, a low-barrier, cross-functional approach to primary care. BMHTH operates three fixed-site syringe service programs and one mobile site, and two clinics offering low-barrier buprenorphine prescribing, across five locations in Southeast Washington State. Everett is the 2021 NASW-WA Public Citizen of the Year. He also serves as Co-Chair for Washington State’s LGBTQ Commission.

Sarah Messmer, MD

Assistant Professor of Clinical Medicine and Pediatrics

University of Illinois at Chicago

Sarah Messmer, MD is an Assistant Professor of Clinical Medicine and Pediatrics at the University of Illinois at Chicago (UIC). She provides primary care and low threshold buprenorphine in partnership with a harm reduction and syringe exchange program run by the UIC School of Public Health, including a mobile van-based program. She is passionate about medical education, with a focus on teaching trainees about the management of substance use disorder, including leading a project to integrate the DATA 2000 waiver into the medical school and residency curriculum at UIC.

PAs

ASAM has been authorized by the American Academy of PAs (AAPA) to award AAPA Category 1.25 CME credit for activities planned in accordance with AAPA CME Criteria. This activity is designated for 1.25 AAPA Category 1 CME credits. Approval is valid until 05/06/2028.  PAs should only claim credit commensurate with the extent of their participation.

Pharmacists
