Overview

This 75-minute on-demand session from the ASAM 57th Annual Conference addresses low-threshold injectable PrEP for people who use drugs, covering pharmacology, access barriers, and harm reduction integration models.

Long-acting injectable PrEP represents a breakthrough in HIV prevention for people who use drugs, offering new opportunities for low-threshold, integrated care. This session combines didactic updates with real-world implementation lessons from leading programs across addiction medicine and harm reduction to help participants translate innovation into clinical practice.

The target audience for this Intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, and students.

This session addresses the following ACGME Competencies: Patient Care and Procedural Skills, Medical Knowledge, Practice-based Learning and Improvement, Systems-based Practice.

This session addresses the following IPEC Competencies:Roles and Responsibilities.

Learning Objectives

Upon completion, learners will be able to:

  1. Describe the pharmacology, dosing schedules, efficacy and safety of longa??acting injectable PrEP agents (cabotegravir and lenacapavir).
  2. Identify key barriers and facilitators to PrEP access and uptake among people who use drugs, including structural, social and systema??level factors.
  3. Discuss real world models for integrating low threshold injectable PrEP into harm reduction and addiction medicine settings (including pregnant/postpartum populations).

I am a Professor of Emergency Medicine at Rutgers New Jersey Medical School and an Associate Director of the Rutgers Addiction Research Center. I am board-certified in Emergency Medicine and Addiction Medicine, with more than two decades of experience designing and leading clinical trials and implementation studies in emergency department and community-based settings. My work focuses on improving access to care for people who use drugs, particularly at the intersection of HIV prevention and opioid use disorder. A major focus of my research has been the development and implementation of low-barrier, emergency department– and mobile-based strategies to expand HIV testing and prevention services, including pre-exposure prophylaxis (PrEP), for hard-to-reach populations. I have led and collaborated on multiple studies examining real-world PrEP delivery in time-constrained clinical environments and have extensive experience partnering with harm reduction organizations, health systems, and public health agencies to support scalable implementation. In parallel, my work in addiction medicine has focused on emergency department–based initiation of medications for opioid use disorder, including studies of high-dose buprenorphine in the context of fentanyl exposure, as well as peer-based and recovery-oriented interventions following overdose. Across these projects, I have led multidisciplinary teams conducting pragmatic clinical trials that integrate rigorous research methods into fast-paced acute care and community settings.

Ruchi M. Fitzgerald, MD, FAAFP is a family physician and an addiction medicine physician. She is the Service Chief of Inpatient Addiction Medicine at PCC Community Wellness, a federally qualified health center system that serves the West Side of Chicago. Dr. Fitzgerald is an Assistant Professor in the Departments of Family Medicine and Psychiatry and Behavioral Sciences at Rush University, as well as the Associate Program Director of the Rush Addiction Medicine Fellowship. She completed the addiction medicine fellowship at Rush University in 2020. She is a graduate of the University of Michigan Medical School and completed her family medicine residency in 2012 with the Montana Family Medicine Residency. Dr. Fitzgerald developed an interest in provision of care for underserved populations and those affected by substance use disorders during her residency. Dr. Fitzgerald also has extensive experience in telemedicine. During her fellowship, Dr. Fitzgerald worked with the addiction medicine faculty to create RISE-MD (Rush Integrative Substance Use Disorder Education for Medical Doctors), an educational initiative in Rush Medical College that focuses on eliminating stigma while promoting evidence-based treatment of substance use disorders in the next generation of physicians. She has worked with the American Society of Addiction Medicine and the Illinois Society of Addiction Medicine on key advocacy and policy initiatives related to substance use disorders. Dr. Fitzgerald is also an active member of the Federation of State Physician Health Programs and is an advocate for physician health and wellness.

Dr. Tani is an Assistant Professor in the Departments of Family Medicine and Psychiatry & Behavioral Sciences at Rush University, as well as core faculty for the Rush Addiction Medicine Fellowship. He is the Assistant Service Chief of Addiction Medicine Consult Service at PCC Community Wellness Center, a federally qualified health center that serves the west side of Chicago. Dr. Tani also sees patients in his family practice clinic, Howard Brown Health, an FQHC on Chicago’s north side where he also provides gender affirming care and HIV treatment.

Dr. Tani’s scholarly and clinical work has focused on advancing low barrier substance use disorder treatment in medically underserved populations, and integrating addiction treatment with primary care in safety net settings. He has taught in local, regional and national settings and enjoys working with interprofessional teams to promote best practices for substance use disorder treatment.

Cayse M. Powell, MD, MPH is a Fellow in Preventive Medicine and General Public Health at Cook County Health and Northwestern University Feinberg School of Medicine. He completed a residency in Obstetrics, Gynecology, and Women's Health at the University of Louisville in 2017 and received an MPH at the Johns Hopkins University Bloomberg School of Public Health in 2025. Dr. Powell is a person with lived experience, and is a physician in long term recovery. He has experience in caring and advocating for patients with SUDs, including pregnant and postpartum individuals and other marginalized populations and prescribing PrEP. He also have significant experience coordinating industry sponsored clinical drug trials. Dr. Powell's clinical and academic interests include maternal substance use disorders, LGBTQ+ health, and infectious disease as it relates to these populations.
