Overview

This 75-minute on-demand session from the ASAM 57th Annual Conference addresses long-acting injectable buprenorphine for birthing and parenting people with OUD, featuring CTN-0080 trial results and person-centered care approaches.

Whole-person SUD care approaches health as an integrated spectrum of disease and wellness that is biological, behavioral, environmental, and social; prioritizes the individual, and efforts to address policy and economic level stressors to improve health. Treatment with LAIB formulations supports access, equity, value-based caring and optimal outcomes. Shared-decision making on choices of MOUD, "complex pain" management, toxicology and breastfeeding is a human right. The workshop will highlight opportunities for providers to be a champion for the health of their patients.

The target audience for this Introductory level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, and students.

This session addresses the following ACGME Competencies: Patient Care and Procedural Skills, Medical Knowledge, Practice-based Learning and Improvement, Interpersonal and Communication Skills, Systems-based Practice.

This session addresses the following IPEC Competencies: Values and Ethics, Communication, Teams and Teamwork.

Learning Objectives

Upon completion, learners will be able to:

  1. Describe evidence and results from CTN-0080 MOMs trial, know models of initiation of LAIB, "complex pain" management and longitudinal tools to improve care for birthing people with OUD.
  2. Utilize ASAM tools and the 4th criteria as a holistic guideline tool to offer person-centered care and choices for MOUD for pregnant, postpartum and parenting people with OUD.
  3. Apply several concepts to improve skills for clinical care of newborns, utility of toxicology testing, and breastfeeding in the setting of non-prescribed opioid use and LAIB formulations.

MD, MSc

Davida Schiff, MD, MSc is a general academic pediatrician and health services researcher focused on understanding how substance use in pregnant and parenting women impacts the health of children and families. She is the Medical Director of the HOPE Clinic (Harnessing support for Opioid and substance use disorder in Pregnancy and Early childhood) at Massachusetts General Hospital, a multidisciplinary program caring for women and families with substance use disorder from the time of conception through the first two years postpartum.

Dr. Schiff completed her undergraduate training at Columbia University, medical training at the Boston University School of Medicine, pediatrics residency in the Boston Combined Residency Program at Boston Medical Center and Boston Children’s Hospital, general pediatrics research fellowship at Boston Medical Center, and master’s program in health services research from the Boston University School of Public Health. Her research is focused on improving care for families affected by substance use and her past scholarship has been published in the New England Journal of Medicine, JAMA, Pediatrics, Academic Pediatrics, JSAT, and Substance Abuse, among other journals. She is an Instructor in Pediatrics at Harvard Medical School.

Marcela Smid MD, MA, MS is a board certified Maternal Fetal Medicine and Addiction Medicine physician at the University of Utah. She has been a member of the Utah Perinatal Mortality Committee since 2016. She is the medical director of the Substance Use & Pregnancy – Recovery, Addiction, Dependence (SUPeRAD) specialty prenatal clinic, a multi-disciplinary clinic for pregnant and postpartum women with substance use disorder. Her research focus is on perinatal addiction, interventions for pregnant and postpartum women with substance use disorders, maternal mortality and maternal mental health.

Melinda "Mel" Ramage, FNP-BC, LCAS, is a family nurse practitioner and licensed clinical addiction specialist. She serves as Director of the North Carolina Perinatal Substance Use Disorder Network and specializes in perinatal SUD systems development. She also practices as an office-based opioid treatment provider with Behavioral Health Group. She is the President-Elect of the North Carolina chapter of the American Society of Addiction Medicine and a U.S. military veteran.

Vania Rudolf

MD, MPH, DFASAM

Vania Rudolf, MD, MPH, DFASAM is a family and addiction medicine physician who is providing care to pregnant and parenting women with substance use disorder. She has a background in family medicine with OB and has completed Fellowships in Integrative Medicine, Addiction Medicine and High Risk Obstetrics at Swedish Medical Center. She has earned public health training at the UW. Dr. Rudolf continues to be a compassionate advocate for improving kind non-judgmental services and removing barriers to care. Providing compassionate trauma-responsive care to vulnerable women, babies and families is the primary focus of her practice in addition to clinical research related to clinical outcomes for substance use disorder, transition to MAT and prescribing policies to optimize safe and adequate postpartum analgesia management. She is the Medical Director for Addiction Recovery Services at Swedish Medical Center, an assistant professor at University of Washington, Seattle, WA and the Chair for the National Womens' Addiction Group related to the American Society of Addiction Medicine. She serves on the maternal mortality review board for the state of Washington and has many years as a practicing clinician.

Tricia Wright, MD MS is a Professor of Clinical Medicine in the Department of Obstetrics, Gynecology and Reproductive Health at the University of California, San Francisco. Previously she was at the University of Hawaii and founded the Path Clinic, a perinatal clinic specializing in the care of pregnant and parenting women with Substance Use Disorders. She is board certified in both Obstetrics and Gynecology and Addiction Medicine and a Fellow of the American College of Obstetrics and Gynecology and a Distinguished Fellow of the American Society of Addiction Medicine. She has published multiple papers on pregnancy and addiction as well as a textbook Opioid Use Disorders in Pregnancy published in 2018 by Cambridge University Press.Dr. Wright completed her undergraduate degree in Biological Sciences from Stanford University, her MD from the University of Michigan. She completed her residency in Obstetrics and Gynecology from the University of New Mexico and obtained a Master’s Degree in Clinical Research from the University of Hawai‘i.

Dr. Ostfeld-Johns, MD, IBCLC is an assistant professor of Clinical Pediatrics and Internal Medicine at Yale University School of Medicine. She graduated from Oberlin College with a BA in Politics, received a Post-bacchalaureate Pre-med certificate from Bennington College, and graduated from University of Rochester School of Medicine and Dentistry with her MD. She moved to Yale for internship and residency in Combined Internal Medicine and Pediatrics, and stayed on as faculty after completion of her training. She earned her IBCLC in 2023.
She now works as a Med-Peds hospitalist — spending half her clinical time taking care of hospitalized adults and half taking care of hospitalized kids, including newborns during their birth hospitalization. She is a medical educator and serves as one of the faculty leaders of the Race, Bias, and Advocacy in Medicine distinction pathway for Internal Medicine residents. She has been working in the area of prenatal substance exposure since 2020 and in that time has collaborated with local partners including pediatric hospital medicine colleagues, child abuse specialists, addiction medicine specialists, obstetrician/gynecologists, social workers, a Yale Law School student clinic, the Connecticut Department of Children and Families, community partners and others locally and nationally committed to improving care for families affected by substance use and substance use disorders.
