Overview
This 60-minute on-demand session from the ASAM 57th Annual Conference addresses drug smoking practices and health risks, including safe handling techniques and harm reduction interventions for clinical settings.
As smoking surpasses injection as the most common route of fatal overdose, clinicians and community providers should understand the emerging risks and harm reduction needs of people who smoke drugs. This interactive workshop explores evolving smoking practices, related health harms, and effective risk mitigation strategies. Participants will engage in hands-on demonstrations and discussions led by clinicians, researchers, and people with lived experience to inform safer smoking interventions and improve care across diverse settings.
The target audience for this Introductory level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, and students.
This session addresses the following ACGME Competencies: Patient Care and Procedural Skills, Medical Knowledge, Interpersonal and Communication Skills.
This session addresses the following IPEC Competencies: Roles and Responsibilities, Communication.
Learning Objectives
Upon completion, learners will be able to:
- Describe current trends in drug smoking practices and associated health risks, including burns, pulmonary disease, and overdose.
- Demonstrate safe handling techniques for smoking devices (e.g. dabbers, pipes) and heat sources (e.g. torches), and apply communication strategies for engaging clients who smoke drugs.
- Identify evidence-based harm reduction interventions and opportunities to integrate safer smoking practices into clinical and community programs.

Honora Englander
MD
Dr. Honora Englander is an Associate Professor of Medicine at in the Division of Hospital Medicine. She completed her BA in Biology at Williams College and her medical degree at University of Massachusetts Medical School. She completed her internship and residency at OHSU in 2006, and joined the faculty at OHSU in 2007. Dr Englander is the co-founder and Director of the nationally recognized Care Transitions Innovation – or C-TRAIN –a hospital-to-home improvement program for uninsured and publicly insured adults at OHSU and that she has spread to three area hospitals through a $17.3M grant from the Center for Medicare and Medicaid Innovations. Her work leading C-TRAIN served as a springboard for healthcare transformation, and led to the development of the Improving Addiction Care Team (IMPACT) at OHSU. IMPACT includes an inpatient addiction medicine consult service and rapid-access pathways to community addiction care. Dr Englander has published her work in JAMA, Annals of Internal Medicine, Journal of Hospital Medicine, and Journal of General Internal Medicine. She sees patients and teaches on the OHSU Medicine wards and at Central City Concern's Old Town Clinic. She is Board Certified in Internal Medicine and Addiction Medicine.
Elizabeth M. Salisbury-Afshar
MD, MPH, FAAFP, FACPM, DFASAM
Elizabeth Salisbury-Afshar, MD, MPH, is a Professor at the University of Wisconsin School of Medicine and Public Health in Madison, Wisconsin. She specializes in family medicine, preventive medicine/public health, and addiction medicine. She is core faculty for the Addiction Medicine Fellowship, providing inpatient and outpatient care at a low barrier walk-in clinic for people who use substances. Dr. Salisbury-Afshar focuses on expanding access to evidence-based addiction treatment and harm reduction services.
Previously, she served as Medical Director at Behavioral Health Systems Baltimore and the Chicago Department of Public Health, and as Director at the Center for Addiction Research and Effective Solutions at the American Institutes for Research. She also served as Medical Director at Heartland Alliance Health, a Chicago-based provider for the homeless.

Professor of Medicine at Boston University Chobanian & Avedisian School of Medicine, HIV primary care physician and addiction specialist at Boston Medical Center
Alexander Y. Walley, M.D., M.Sc., (he/him/his) is Professor of Medicine at Boston University Chobanian & Avedisian School of Medicine and HIV primary care physician and addiction specialist at Boston Medical Center whose research is focused on overdose prevention. He is the director of the Clinical Addiction Research and Education Unit. He is past president of the American College of Academic Addiction Medicine. He was a founding director of the Grayken Addiction Medicine Fellowship Program at Boston Medical Center from 2011-2022. With colleagues, he founded Boston Medical Center’s inpatient addiction consult service and walk-in substance use care bridge clinic. He serves as the medical director for the Massachusetts Department of Public Health’s Bureau of Substance Addiction Services, the Overdose Prevention Program, and the SafeSpot Overdose Prevention Hotline.
