Overview
This 75-minute on-demand session from the ASAM 57th Annual Conference addresses extended-release buprenorphine effectiveness in OUD, presenting NIDA CTN study results for pregnant patients and emergency department settings.
Presentations from two large, HEAL-funded, NIDA CTN multi-site trials: 1) The Medication Treatment for Opioid-dependent Expecting Mothers (MOMs) Trial showed that participants who received extended-release buprenorphine had significantly higher illicit opioid abstinence during pregnancy and was non-inferior postpartum. 2) The Emergency Department-INitiated bupreNOrphine and VAlidaTIOn Network Trial (ED-INNOVATION) study showed successful implementation of extended release (XR)-BUP in ED patients with untreated OUD; and that it was superior to sublingual (SL) BUP in linkage to care post-ED induction.

Learning Objectives

- Describe the role of extended-release buprenorphine in the management of pregnant and post-partum women with OUD
- Identify patients with OUD eligible for successful induction on extended-release buprenorphine when they present at ED settings
- Discuss the evidence for the safe and effective use of extended-release buprenorphine in two high risk populations

Geetha A. Subramaniam, MD, DFAPA, DFAACAP is the Deputy Director of the Center for Clinical Trials Network, at NIDA. In this role, she has been instrumental in developing research projects in a variety of topic areas, including addressing the prevention of substance abuse in adolescents, prevention and intervention of prescription stimulant misuse in youth. Previously, she was a full-time faculty member in the Division of Child Psychiatry at Johns Hopkins School of Medicine and the Associate Medical Director of Mountain Manor Treatment Center, Baltimore, MD, where she led clinical trials with adolescents with SUD and served as a preceptor to residents and fellows. She distinguished herself as a clinical and research expert in the assessment and treatment of adolescents and young adults with opioid use disorders. She is ABPN certified in General, Child, and Adolescent, and Addiction Psychiatry and a Distinguished Fellow of both the American Psychiatric Association and the American Academy of Child and Adolescent Psychiatry.

Gail D'Onofrio
MD, MS, FACEM, FASAM
Dr. D’Onofrio is Professor and the Inaugural Chair of the Department of Emergency Medicine at the Yale School of Medicine (2009) and Physician-in Chief of Emergency Services at Yale-New Haven Hospital EDs with an annual census of approximately 180,000 patients. Dr. D'Onofrio is also Professor in the School of Public Health in the Department of Chronic Disease Epidemiology. She is boarded in emergency and addiction medicine. Dr. D’Onofrio has extensive experience as a leader, researcher, mentor and educator, and is internationally known for her work in alcohol and other substance use disorders (SUDs) as well as for her research on gender variations in women with ischemic heart disease. Her work (JAMA, 2015) demonstrating that ED-initiated buprenorphine increases engagement in addiction treatment for individuals with OUD, has changed clinical practice, receiving multiple science awards, including awards from the Hazelden Betty Ford Foundation and the Clinical Research Forum.
Dr. D’Onofrio has a long track record of mentoring physician scientists in independent research careers. She is the PI of a NIDA K12 establishing the Yale Drug Use, Addiction and HIV Research Scholars (Yale-DAHRS) program, a Mentored Career Development Program with focused training in prevention and treatment of drug use, addiction, and HIV in general medical settings with scholars in Medicine, Emergency Medicine (EM), Pediatrics, Obstetrics and Gynecology, and Pulmonary Critical Care; and she has mentored numerous EM faculty.
She has received multiple awards including Excellence in Mentoring award from the Association for Medical Education and Research in Substance Abuse (AMERSA 2008), Advancing Women in Emergency Medicine award from the Society of Academic Emergency Medicine (SAEM 2016) and the Academy for Women in Academic Emergency Medicine (AWAEM) Outstanding Department Award for the advancement of women (SAEM 2018). She is also the recipient of the ASAM 2021 R. Brinkley Smithers Distinguished Scientist Award.
