Decreasing Barriers to Methadone Access: Lessons from Jail and COVID-19

Overview

This 1-hour on-demand session from ASAM Virtual 2021 addresses the barriers to access to methadone to treat incarcerated populations, reviews the steps for correctional facilities becoming an OTP, and discusses the impact of COVID-19.

The majority membership in ASAM is not affiliated with a federally licensed OTP. Regulations for OTPs were written in 1972 and continue to be the scaffolding for methadone administration in the United States. This presentation will explore the conflict between bringing methadone treatment to those incarcerated and those in rural regions when regulations at the federal (42 CFR 8) and state level are arduous and rigid. Our small county jail in western Massachusetts achieved full licensure as an OTP in August 2019. This presentation will review steps for becoming an OTP, including a "how-to" for correctional facilities considering obtaining an OTP license. It will query regulatory infrastructure amidst the opioid epidemic, discuss the loosening of regulations during COVID-19, and examine the increased vulnerability of OTPs.

The target audience for this intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, students, and counselors.

This session addresses the following ACGME Competencies: Systems Based Practice.

Learning Objectives

Upon completion, learners will be able to:

1. Outline the steps needed to obtain an OTP license as a correctional facility
2. Recognize the different carceral systems in the United States and how medical care is delivered in those settings
3. Summarize the history of federal oversight in methadone administration

2. Click "Complete Post Test" to answer quiz questions. You will have 10 attempts and must get at least 2 out of 3 questions correct. After completing the quiz, return to the Contents tab. 
3. Click Complete Evaluation to answer evaluation questions. Scroll down on all questions, there are answers that expand past the size of the window. Submit the evaluation (the next box should activate, and the evaluation box should turn green with a white check mark).
4. Click the button “Claim Credits” in the box titled “Claim Credits & Certificate." Choose the type of credit and click submit. Click the button “View/Print Certificate” to save or print your certificate. If you ever lose your certificate, you can come back to the ASAM e-Learning Center and view it on your transcript (found in the Dashboard).
