Overview

This 75-minute on-demand session from the ASAM 57th Annual Conference addresses clinical and ethical challenges for patients with addiction and cancer, exploring pain management and care decisions in complex scenarios.

Participants in this interactive session will explore cases at the challenging intersection of addiction, pain, and cancer. Clinicians boarded in addiction and palliative medicine guide participants through clinical tools and scenarios that: prioritize options for managing concurrent addiction and cancer pain, approach a differential and treatment options for chronic cancer pain and prescription OUD versus OUD, and consider ethical responsibilities in addiction and cancer treatment when patient autonomy conflicts with clinical care in the 'best interest' of the patient.

The target audience for this Intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, and students.

This session addresses the following ACGME Competencies: Medical Knowledge, Interpersonal and Communication Skills.

This session addresses the following IPEC Competencies: Values and Ethics, Communication.

Learning Objectives

Upon completion, learners will be able to:

  1. Apply a multi-dimensional assessment/ 4P framework to help prioritize more appropriate and safer opioid options in patients with pain, cancer, and addiction
  2. Compare features and management approaches for chronic cancer pain and OUD or prescription OUD or Complex persistent opioid dependence
  3. Examine ethical and clinical considerations when patients with untreated OUD and cancer decline medications for OUD, cancer treatment, and resuscitation

Janet J. Ho

MD, MPH

Janet Ho, MD MPH is a board-certified palliative medicine and addiction medicine physician at the University of California, San Francisco. She completed internal medicine training and chief residency at Yale, and fellowships in health services research, palliative care, and addiction medicine at Harvard and Massachusetts General Hospital in Boston, MA. Her clinical and research interests lie at the intersection of serious illness, the disease that is addiction, pain, and chronic cancer pain. Dr. Ho is dedicated to improving provider knowledge and confidence in primary palliative care and addiction medicine; improving disparate quality of life and care for patients with serious illness and addiction; understanding the role of buprenorphine in palliative care; and challenging stigma against patients who use drugs.

MD

Katrina Nickels is a palliative care and addiction medicine physician at the University of Kentucky in Lexington, KY. She did her medical training at the University of Kentucky, and is board certified in Internal Medicine and Hospice and Palliative Medicine.  She worked in hospice and palliative care for 12 years with Bluegrass Care Navigators.  She joined ASAM in 2018 and has been a board member of the KY chapter since 2020.  She graduated from formal training in the addiction medicine fellowship in the UK in 2022. Her clinical interests are the intersection of pain, palliative care, substance use, and serious medical illness.

A. Justine Landi, MD, MPH

University of Chicago

Andrea "Justine" Landi, MD, MPH is an Assistant Professor of Medicine at the University of Chicago in the Section of Geriatrics and Palliative Medicine and is faculty at the MacLean Center for Clinical Medical Ethics. She is a clinician educator triple-boarded in family medicine, geriatrics, and palliative medicine with additional training in addiction medicine and clinical medical ethics. She practices in the post-acute and long-term care, primary care, and inpatient settings. She provides comprehensive care for older adults that focuses on the management of multiple chronic medical conditions, safe transitions of care, and substance use disorders. Her scholarly interests are focused on person-centered substance use disorder care of older adults across healthcare settings using ethical frameworks. She was awarded the HRSA Geriatrics Academic Career Award in 2023 through which she will promote interprofessional collaboration across care settings, specifically nursing homes, in providing equitable access to age-friendly, evidence-based dementia and opioid use disorder care for older adults.

MD

Julie Childers is an addiction medicine and palliative care physician at the University of Pittsburgh, where she developed and now directs the Addiction Medicine Fellowship. She has written and taught nationally in the areas of managing addiction in individuals with serious illnesses, communication, motivational interviewing, and medical ethics. In 2021, she was elected to the University of Pittsburgh School of Medicine's Academy of Master Educators.
