Overview
This 75-minute on-demand session from the ASAM 57th Annual Conference addresses novel kratom-derived products including 7-OH and kava, covering pharmacology, clinical assessment, and evolving regulatory landscapes.
Gas station heroin?" and "legal morphine?" Think again! In this session we myth-bust and dismantle the pejorative language used to describe kratom-derived products consumed by millions of adults. This session synthesizes epidemiological findings, real-world case reports, preclinical literature, and emerging data to inform attendees about kratom-derived products relevant to their practice. These include products containing 7-hydroxymitragynine ("7-OH"), mitragynine pseudoindoxyl ("pseudo"), kratom-kava products, and emerging MGM analogues. Recommendations are given for how substance use disorder assessment, diagnosis, and treatment can improve. This workshop is an interactive session that challenges participants to examine the real-world barriers in PSUD care in their own community and leave with strategies to build systems change. Participants will participate as small groups to learn more about the North Carolina PSUD Network and use the evolution of the NC PSUD Network as their blueprint to identify strategies to improve PSUD access and collaboration in their own state.
The target audience for this Intermediate level session includes physicians, nurse practitioners, physician assistants, other clinicians, researchers, residents, fellows, and students.

Learning Objectives
Upon completion, learners will be able to:
- Describe the basic pharmacology, toxicology, effects, risks, and benefits associated with kratom and kratom-derived products.
- Conduct highly specified clinical assessments, as well as better diagnosis and manage SUDs related to kratom-derived products in a data-informed and patient-centered manner.
- Define the dynamic state and federal policy and regulatory landscape surrounding kratom and kratom-derived products.

Katherine (Katie) Hill is a PhD candidate in Epidemiology of Microbial Diseases at Yale School of Public Health. Her research interests include substance use and harm reduction. Her doctoral research employs mixed methods to evaluate the impact of emerging substances, such as xylazine and kratom, on people who use drugs.

Kirsten Smith
PhD, MSW
Dr. Kirsten Smith is currently a postdoctoral fellow at the National Institute on Drug Abuse Intramural Research Program in the Translational Addiction Medicine Branch and the Real-world Assessment, Prediction, and Treatment Unit. After receiving her Master’s degree in clinical social work from the University of Kentucky and working directly with people with substance use disorders, Dr. Smith pivoted to a scientific career to better understand problems she observed in the clinic. Dr. Smith began investigating kratom in 2017 and is now studying its use in real-world settings, using survey and social-media analyses. Dr. Smith is extending this to include ecological momentary assessment and kratom product assay, which can inform an interdisciplinary line of laboratory studies ranging from observational work to randomized behavioral-pharmacology experiments. Dr. Smith serves as the Lead Associate Investigator on a double-blind randomized placebo-controlled trial examining the withdrawal-suppression efficacy of a biased agonist in humans with opioid-use disorder. Concurrently, Dr. Smith is developing protocols for studying kratom, which she plans to continue as she transitions into a tenure-track university position.
