    Overview

    This 8-hour virtual-live course expands on the content discussed within the ASAM Criteria One-Day Foundations course and provides an in-depth understanding of developing individualized treatment plans, evaluating initial reviews and continued stay reviews, and determining when it is appropriate to initiate transfer or discharge of a patient from treatment.  

    The ASAM Criteria Skill Building Course is designed to help healthcare professionals develop a holistic understanding of The ASAM Criteria and explore their role in constructing a full range of services for the treatment of individuals with substance use disorders (SUD). The Course builds on the foundational concepts of The ASAM Criteria that will offer practical guidelines to drive objective decision making and ensure fidelity to The ASAM Criteria. Using a practical and case-based approach, participants will explore important considerations in developing individualized treatment plans, evaluating progress versus nonprogress in treatment, and determining when it is appropriate to initiate transfer or discharge of a patient from treatment. Further, the training will provide tools and resources to equip healthcare providers to work effectively in managed care settings and explore how case shaping can drive patients' overall quality of care.

    The target audience for this introductory, intermediate, and advanced course include: counselors, social workers, payers, administrators, and other clinical staff.

    Learning Objectives

    1. Examine the guiding principles of the ASAM Criteria and the payer’s role in its system-wide implementation
    2. Evaluate patient risk ratings across the six dimensions based on imminent danger and immediate needs, as defined in the ASAM Criteria
    3. Evaluate treatment plans and clinical assessments to determine if the requested level of care is appropriate in accordance with the ASAM Criteria
    4. Conduct objective, comprehensive clinical case reviews (and re-reviews) based on the ASAM Criteria to drive ongoing evidence-based care
    5. Implement a collaborative process between treatment providers and payers using common terminology of the ASAM Criteria to ensure patient-centered care
    6. Examine implementation challenges and develop strategies to provide appropriate treatment for patients with SUDs

    Tiffany Lee

    PhD, LPC, CAADC

    Dr. Tiffany Lee is a Professor at Western Michigan University(WMU) and the Director of the Specialty Program in Alcohol and Drug Abuse. From21-23, Dr. Lee was the Chair of the School of Interdisciplinary Health Programs at WMU. She has over 15 years of experience in the field of addiction, primarily as a counselor and court advocate. Her grant activities and researchinterests primary focus on addiction training in various medical and mental health professions.

    Dr. Lee recently published a second edition of a textbook on diversity issues in addiction treatment and has served on the Michigan Health Professionals Recovery Committee. In that capacity, she worked closely working with the Attorney General's office for several years to update policies for the recovery program in the state. Dr. Lee received her B.S. in Psychology from the Univ. of Florida, M.A. in Community Counseling from the Univ. of Alabama at Birmingham, and Ph.D. in Counselor Education from WMU.  

    She enjoys spending time with her young son and partner, especially when it includes any time on the water or camping. She is also a huge college football fan.

    Joseph Kuchenbuch

    MA, LLP, CAADC, CCS-M

    Joe has worked in field of clinical mental health and substance use disorder services for the past 36 years and the juvenile justice system for 8 years previously. He possesses a Bachelor of Arts in Psychology from Albion College and a Master of Arts degree in Counselor Education and Counseling Psychology from Western Michigan University. Additionally, he is a Certified Advanced Alcohol and Drug Counselor and a Certified Clinical Supervisor through the Michigan Credentialing Board for Addiction Professionals.

    Joe is currently the Director of Behavioral Health Services at Western Michigan University. In addition to his administrative role, Joe provides clinical supervision as well as providing assessment, group, and individual counseling services. During his career, Joe has worked with both the adolescent and adult populations and more recently he worked closely with a member of the WMU/Unified Clinics to establish a Dog-Assisted Therapy clinic.

    Joe resides in Portage, Michigan with his spouse of 28 years and together they raised two adult sons. He is an avid snow skier, scuba diver, and is currently trying to learn how to play a social game of golf after years of not playing due to school and work.

Learners will have the opportunity to ask the presenter questions in real time, participate in polling questions and break into small groups. 

    The ASAM Criteria Course can also serve as a bridge to the implementation of a provider credentialing program. Providers who have completed ASAM approved training can gain the initial skill set to deliver care in compliance with The ASAM Criteria.

    Overview

    This 6-hour, live course provides the education needed for prescribing clinicians to strengthen skills in partnering with treatment court and advocating for patients who are involved in treatment court. The course will include didactive presentations, small group discussions, FAQs, and challenging case studies. 

    ASAM is partnering with All Rise through an Office of National Drug Control Policy (ONDCP) grant initiative to allow medical providers active with and new to the drug treatment court field access to training to help guide them in successful integration of services for individuals with substance use disorders involved in the justice system.

    The target audience for this introductory level activity includes: physicians, physician assistants, and nurse practitioners who practice addiction medicine and interact with treatment courts and other aspects of the justice system. 

    This activity addresses the following ACGME Competencies: Patient Care, Medical Based Knowledge, Practice Based Learning, Interpersonal and Communication Skills, Professionalism and Systems-based Practice.

    Learning Objectives

    Upon completion, learners will be able to:

    1. Describe ways to reduce stigma against people with substance use disorders, including the use of accurate terminology. 
    2. Explain the role of timely and accurate diagnosis and treatment for clients/participants first engaging in treatment courts.
    3. Define addiction as a chronic and manageable disease.

    David Streem

    MD, FASAM

    David W. Streem, M.D. is a staff psychiatrist and Medical Director of the Alcohol and Drug Recovery Center at the Cleveland Clinic in Cleveland, Ohio. He is certified both in psychiatry and psychosomatic medicine by the American Board of Psychiatry and Neurology. He has been a diplomate of the American Board of Addiction Medicine since May 2009. He has been a certified Medical Review Officer since 2005. He has been the Quality Review Officer in the Department of Psychiatry and Psychology since 2011, and the Neurological Institute’s Compliance Officer since 2015. He is a Clinical Assistant Professor of Medicine in the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University and previously served as Co-Course Director for the Neuromuscular Sciences curriculum. Dr. Streem held the position of Adult Psychiatry Residency Program Director from 2007 to 2009.

    Soumya Pandalai

    MD, FACP, DFASAM

    Dr. Soumya L. Pandalai is a board certified physician in the field of Internal Medicine and Addiction Medicine. She currently works at Banner University Medical Center Phoenix (BUMCP) in Phoenix, Arizona as a specialist in Addiction Medicine and provides both inpatient consultation services and outpatient treatmentr to persons with Substance Use Disorders (SUDs). Dr. Pandalai enjoys the clinical practice of Addiction Medicine and finds the neuroscience of addiction to be intellectually stimulating and challenging to observe the health-effects that can result from various aspects of human behavior. It is also immensely rewarding to connect with patients at different stages of their recovery process and help them along their path toward healing. She hopes to inspire people to view addiction from a different lens and apply principles of substance use and reward pathways to general aspects of human motivation and other medical illnesses. Dr. Pandalai is proud and grateful to work with an interdisciplinary team of hospital staff, social workers, counselors and other medical providers to create a more inclusive and health-focused model for substance use treatment, chronic pain and use of treatment options like tele-health to advance the care of vulnerable patient populations.

    Steve Hanson

    MS Ed

    Steve Hanson, M.S. Ed., is a Senior Treatment Consultant for AllRise/Treatment Court Institute. Having been a faculty member for AllRise (founded as National Association of Drug Court Professionals in 1994) for over 20 years, Mr. Hanson began his current role in January of 2023. 

    Previously, he served as the Associate Commissioner for Courts and Criminal Justice for the New York State Office of Alcoholism and Substance Abuse Services. He was responsible for relationships with the various criminal justice entities including Drug Courts, State and local corrections, probation and parole. Previously he had served as the Associate Commissioner of Treatment for OASAS with responsibilities for the oversight of state operated treatment programs, adolescent services, criminal justice and Recovery services.

    He taught undergraduate and graduate level courses in Psychopharmacology at SUNY Brockport for 20 years.

    Steve has over 40 years of experience in the field as a treatment provider, educator and consultant. His ability to train criminal justice practitioners on drug abuse and treatment has been recognized both nationally and internationally.

    Jihan Judeh

    LMFT

    Jihan is a licensed Marriage and Family Therapist (LMFT 51292 CA), who has worked in the mental health field for over 16 years, with the last 11 years focused primarily on addiction treatment. She has served as both a Clinical Director and Clinical Supervisor for different levels of care that implemented the ASAM Criteria including Outpatient, Intensive Outpatient and Residential treatment services. Through her work in addiction treatment, Jihan has seen the importance of meeting clients where they are in their recovery journeys and treating the whole person and not just the addiction. 


    In addition to being a member of ASAM, Jihan is also a member of CAMFT (California Association of Marriage and Family Therapists). She was a contributing member to The ASAM Criteria 4th Edition, working on the Treatment Planning Writing Committee as well as serving as a Dimensional Admission Criteria Case Tester. Jihan currently provides telehealth mental health services to couples and individuals while also serving as a faculty for ASAM.

    Jihan earned her master’s degree in Marriage and Family Therapy from California State University, Long Beach in 2007 and became a licensed therapist in 2012. She enjoys spending time with her husband and two children, particularly enjoying boating and camping with her family.

    Michelle Niculescu

    LPC

    Michelle has been working in the field of addiction for over 20 years, Michelle dedicates her career to taking a translational approach to addiction medicine. Her work spans from the molecular underpinnings of addiction to behavioral interventions. 

    Her endeavors include basic science laboratories, clinical psychology, higher education, and counseling. She currently works as a Clinical Algorithm Scientist for ASAM.  

    Michelle earned her doctorate in pharmacology from Lewis Katz School of Medicine at Temple University and her MA in clinical mental health counseling from University of the Cumberlands. She is a licensed professional counselor in Pennsylvania. She resides in Pittsburgh with her 3 kids, 2 dogs, and 1 husband. She enjoys running and yoga.

    The Efficacy of GLP-1 Agonists in Treating Substance Use Disorder in Patients A Scoping Review

    Overview

    This one-hour, on-demand, journal article-based activity presents a scoping review on the efficacy of GLP-1 agonists in the treatment of patients with substance use disorder. While there is interest in exploring the use of GLP-1 agonists in the treatment of SUD, evidence for safety and efficacy of GLP-1 agonists in humans is limited. The results of this scoping review are mixed, and the authors call for further research to determine the efficacy of GLP-1 agonists in treating SUD.

    The target audience for this intermediate continuing education activity includes: physicians, nurse practitioners, PAs, pharmacists and other clinicians, researchers, students, and policymakers.

    Article Abstract

    Background
    Substance use disorder (SUD) continues to be a leading cause of morbidity and mortality with limited treatments. There is interest in expanding the use of GLP-1 agonists in treating SUD. However, evidence for safety and efficacy in humans is limited. This review aims to bridge the existing knowledge gap by establishing a baseline of literature in this area to inform future trials and clinical practice. Our inclusion criteria were English peer-reviewed manuscripts reporting on use of GLP-1, GIP, and/or glucagon receptor agonists in treatment of SUDs, excluding case studies. The literature search was performed in accordance to PRISMA guidelines. Five studies were included in this review examining the use of this medication in tobacco use disorder, alcohol use disorder, and cocaine use disorder. No studies regarding substance withdrawal syndrome were identified. The included studies varied widely in terms of patient selection, dose/formulation of GLP-1 agonists, and follow-up. The results of this scoping review are mixed, with 3 studies demonstrating positive results and 2 studies finding no efficacy of this medication on SUD outcomes. It is premature to prescribe this medication off-label to patients. Further research is needed to determine the efficacy of GLP-1 agonists in treating SUD.

    Methods
    The search algorithm was developed with a medical librarian to search PubMed and APA PsychINFO from database inception through October 19, 2023. Search terms included the different forms of GLP-1, GIP, and glucagon receptor agonists including trade names along with SUDs by each individual substance (ie, alcohol, opioid, cocaine, stimulant, sedative, hallucinogenic, tobacco). For purposes of this review, all forms of GLP-1 agonists, including GIP/GcgR co-agonists, will be referred to as GLP-1 agonists, unless specified. Studies were identified and included, abiding by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines.13 References were extracted and imported into Zotero, a reference manager. Duplicate articles were identified and removed. Each article was assigned a unique record number.

    Results
    A total of 308 studies were identified in the initial search, with 11 studies passing the title and abstract screen (Fig. 1; PRISMA). Six studies did not meet criteria. Two studies were excluded due to not being peer reviewed, 2 were excluded due to incorrect study design, and 2 were excluded for delineating a proposed protocol without results reported. Thus, in the full text review, a total of 5 studies met the criteria (Table 1). Of these 5 studies examining the effects of GLP-1 agonists in SUDs, 2 investigated tobacco use disorder, 2 studied AUD, and 1 examined cocaine use disorder. No studies regarding substance withdrawal syndrome were identified.

    Discussion
    This review explores the evidence base for using GLP-1 agonists, traditionally utilized in metabolic disorders, as a novel approach to treat SUDs. Given the interest in expanding the use of these medications outside of metabolic disease, it is important to understand the existing evidence to inform clinical decision-making and identify gaps in the literature in humans.

    The Impact of High Potency Synthetic Opioids on Pharmacotherapies for Opioid Use Disorder: A Scoping Review

The interactions between HPSOs and MOUDs are not yet well understood. This review examines the relationship between HPSO exposure and patient outcomes when using common MOUD treatment protocols. It also discusses the implications for clinical practice and suggests directions for future research based on the current research on patient populations impacted by HPSOs.

    Article Abstract

    Background
    The clinical implications of high potency synthetic opioids (HPSO) on medications for opioid use disorder (MOUDs) are not well understood. Although pharmacological interactions are plausible, the clinical significance of such interaction has not been systematically elucidated. This scoping review investigates the relationship between HPSO exposure and various MOUD treatment outcomes.

    Methods
    We followed PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses Extension for Scoping Reviews) for scoping reviews with extensive a priori search strategy of databases: MEDLINE, EMBASE, PsycINFO, Web of Science, CINAHL, and Cochrane.

    Results
    From 9149 studies, 34 fulfilled the inclusion criteria. Synthesized data reveal several critical insights: First, there is a variable but high occurrence (38%–80%) of HPSO usage among individuals with MOUDs. Second, MOUDs are linked to a decreased risk of overdoses and deaths associated with HPSO. Third, HPSO consumption is correlated with the risk of precipitated withdrawal when starting buprenorphine. Fourth, low-dose buprenorphine is being recognized as one method to avoid moderate withdrawal symptoms prior to treatment. Lastly, significant gaps exist in human experimental data concerning the effects of HPSO on key factors critical for treating OUD-craving, withdrawal symptoms, and pain.

    Conclusions
    Current evidence supports MOUD safety and effectiveness in reducing nonmedical opioid use. Further research is needed to explore HPSO’s influence on the acute factors preceding nonmedical opioid use, such as cravings, withdrawal symptoms, and pain. This research could inform the optimization of MOUD dosing strategies. Achieving consensus and harmonizing data across clinical and research protocols could diminish variability, enhancing our understanding of HPSOs effect on MOUD treatment outcomes.

    Overview

    This 6-hour, online course series consists of 6 modules that cover the foundations of pain and addiction including the science, stigma, screening and assessment, treatment, and interdisciplinary approaches. The modules are self-paced and include interactive activities, knowledge check questions, video content, and text-based content. All modules can be taken together as a full 6-hour course or can be taken individually to fill learner knowledge gaps in pain and addiction topics. 

    The target audience for this introductory series includes: primary care physicians, fellows in training and residents, providers who are new to addiction medicine or are early in their career and other members of the care team who are looking for introductory topics on pain and addiction.

    This series addresses the following ACGME competencies: Patient Care, Medical Knowledge, Practice Based Learning, Interpersonal Skills, Professionalism, and Systems Based Practice.

    DEA Education Requirement

    As an accredited organization in Section 1263 of the Consolidated Appropriations Act of 2023, ASAM certifies that completion of this course meets 6 hours of the DEA requirement for 8 hours of education on substance use disorder(s).

    Overview

    This 1-hour online module will discuss options for nonpharmacological treatment of pain and addiction, including interventional treatments, behavioral therapy, physical and occupational therapy, digital apps, 12-step programs, and other non-pharmacological treatment options for patients. This is the fifth of six modules in The ASAM Pain & Addiction Essentials series.  

    Treating pain and addiction is a complex topic that many providers struggle to understand. This module will focus on nonpharmacological treatment options while the previous module, Module 4, focuses on pharmacological treatment options. Together, these two modules will provide guidance for practitioners to develop evidence-based treatment plans for patients with pain and addiction in alignment with the patient’s treatment goals, communicate with the patient about the treatment plan and manage and monitor patient progress effectively. Specifically, Module 4 will discuss nonpharmacological treatment of pain and addiction, including interventional treatments, behavioral therapy, physical and occupational therapy, digital apps, 12-step programs, and other non-pharmacological treatment options for patients.

    Learning Objectives

    Upon completion, learners will be able to:

    1. Explain the rationale for developing a multimodal treatment plan to the treatment of pain and addiction, which incorporates non-pharmacological approaches.
    2. Explain how to use patient-centered communication techniques, such as those used in motivational interviewing, to support and sustain patient engagement and motivation in treatment. 
    3. Explain how nonpharmacological interventions and therapies employed by behavioral health therapists, occupational therapists, and physical therapy specialists  as well as other kinds of interventions are effective and powerful components in the treatment of pain and addiction.
    4. Discuss considerations when making referrals to other specialists/providers for treating pain and addiction or promoting functional well-being.

    Donald R. Teater

    MD, MPH

    Don Teater MD, MPH is a family physician who has worked in western North Carolina for the past 30 years. Since 2004 he has focused on the intersection of pain, opioids, and addiction. Don was the lead facilitator for the expert panel during the development of the CDC Guidelines for Prescribing Opioids for Chronic Pain.

    From 2013-2016, Don was the Medical Advisor at the National Safety Council leading their effort to reduce opioid abuse and overdose in the community and in the workplace. He continues to work with federal and state government organizations, medical organizations and nonprofits in addressing many aspects of the opioid epidemic.

    While working on the public health aspects of the opioid epidemic, he sees patients one day a week treating opioid use disorder and chronic pain in Waynesville, NC by telemedicine. He also works one week each month as a pain and addiction specialist seeing Alaska Native patients with the SouthEast Alaska Regional Health Consortium (SEARHC) in Juneau, AK.

    Launette Marie Rieb

    MD, MSc, CCFP, FCFP, DABAM, FASAM

    Launette Rieb, MD, MSc, CCFP, FCFP, DABAM, FASAM is a Clinical Associate Professor in the Department of Family Practice at the University of British Columbia (UBC), Canada. She is a Family Physician and diplomat of The American Board of Addiction Medicine. She did her graduate work in the area of pain physiology. She completed a postgraduate UBC Clinical Scholar's Program in 2015 and a NIDA sponsored Canadian Addiction Medicine Research Fellowship in 2016 resulting in publication on a newly described opioid pain phenomenon - withdrawal-associated injury-site pain (WISP). She has also published on fentanyl and heroin overdose deaths in BC, as well as on addiction in a variety of marginalized populations. Dr. Rieb is the Medical Director of a multidisciplinary team at OrionHealth (Vancouver Pain Clinic), and works as a consultant for the Rapid Access Addictions Clinic at St. Paul’s Hospital in Vancouver. In addition, she does addiction medicine consultations for The Orchard Recovery Centre, on Bowen Island. Dr. Rieb has taught addiction medicine in the undergraduate and postgraduate medical programs at UBC, and at national and international conferences for 24 years. Dr. Rieb was the co-creator and initial Physician Director of the St. Paul’s Hospital Goldcorp Addiction Medicine Fellowship (now the BC Centre on Substance Use Addiction Medicine Fellowship). She is a member of the Canadian Society of Addiction Medicine’s Education Committee and the College of Family Physician of Canada's Competency Creation Working Group for the Certificate of Added Competency in Addiction Medicine. Dr. Rieb is the past recipient of a UBC Faculty of Medicine Post Graduate Teaching Award.
